Intention Health is working to develop 'CheckUp Book'. A tool to help patients take ownership of their own medical data and records to better integrate care between health professionals and improve health outcomes. 

Sometimes the health system seems to fear patient empowerment. At Intention Health we embrace it and are working on a tool that allows them to be in control of their own health data allowing them to guide their own treatment and health outcomes.
